How your information is used

This independent guest-pass inquiry page is managed locally by a City Sports Club Newhall Drive sales representative. It is separate from the official City Sports Club corporate website and is not automatically linked to its Guest Tracking system.

What we collect

When you submit a request, we collect your name, phone number, email address, the guest-pass option you selected, and an optional source label used to identify how you found the page. The form host may also process technical information for security and anti-spam protection.

Why we use it

We use your information to respond to your guest-pass inquiry and help arrange eligibility and redemption. A representative may manually enter your information into City Sports Club's Guest Tracking system to help fulfill your request. Submitting does not authorize automated marketing texts or unrelated promotional contact.

Storage and sharing

Form responses are stored in a privately controlled inbox provided by Netlify. Verified requests are also copied automatically to a private Google Sheet for local follow-up. Only authorized administrators can access these records. Information may be shared with the club for follow-up related to the pass. We do not sell submitted contact lists.

Correction, removal, and questions

To request correction or deletion of a submission, contact the representative who provided the link, or inquire in person at 610 Newhall Drive, San Jose. For general club questions call (408) 490-2164. We review records regularly and delete them when no longer needed for follow-up, subject to approved business policies.

Guest-pass conditions

Requests are for eligible nonmembers age 18 or older. Final terms, eligibility, redemption and amenity access must be confirmed by the club. This form is a request, not an automatically issued guest pass.
